2026-2028 Strategic Plan

The Board of Directors of the Summerville Family Health Team is pleased to present
the 2026 – 2028 Strategic Plan!

Summerville Family Health Team was established in 2007, as one of the first Family Health Teams in Ontario, and currently supports more than 50,000 people in Mississauga, Southwest Etobicoke and the surrounding communities. Summerville offers primary care through a diverse interdisciplinary team including physicians, nurse practitioners, registered nurses, social workers, dietitians, clinical pharmacists, health promotion specialists, a physiotherapist, a kinesiologist and a variety of other health care professionals and administrators working across five clinical practice sites, to provide key services with a focus on underserved and marginalized people (elderly, low socio-economic, ethnically diverse).

In early 2025, the Summerville Board formed a Strategic Planning subcommittee to develop our fourth Strategic Plan, which will cover the period from 2026 to 2028. The new Strategic Plan is intended to help align all members of the Family Health Team and to reinforce the importance of the organization’s work, the value of its offerings and its commitment to the community.

The process for the development of the new Strategic Plan included a variety of focus groups with a selection of staff and physicians from all five sites, an online survey for all team members, external outreach including patient focus groups and an online survey, and external stakeholder interviews. We are grateful for the input we received from our patients, other community members, volunteer Board members, physicians and staff.

The consensus from the input gathered was that our Vision remains relevant, that Summerville will be “Vital to the Health and Well-being of the Community”. Our Mission has been simplified to “Provide People-Centered, Community-Based Primary Care”, and in so doing so we will:

Advance primary care, prevention and treatment
Strengthen partnerships and system-wide collaborative care
Educate and train future clinicians
Support each other as team members.
